24 hour telephonic self-service

You can now request various items via our new and convenient 24 hour automated telephonic self-service facility.
Simply dial 0860 00 4367 and follow the voice prompts to:

Change your benefit options (only once a year, before the cut-off date);

Change your contact telephone and fax numbers;

Request tax certificates;

Request new member cards and

Request the following forms:

Member application form;
Chronic medication application form;
PMB programme application form;
Affidavits confirming dependency;
Maternity Programme enrolment form or
Newborn registration form.

Ensure that you have your member and ID numbers handy for any requests, except when you are requesting forms.
